The Rev. Bill Crews Foundation provides direct assistance to address the cause and effect of homelessness, poverty and disadvantage. We succeed in achieving this through caring services including food provision, social welfare and education support.

Our Social Health and Wellbeing team are the first point of contact for people facing multiple complex challenges in their lives. They offer a range of support from crisis support, case management, essential needs assistance including bills assistance, medical, dental and complimentary medical services.

THE ROLE

As an AOD Clinician, you will be providing assessment, case management, counselling, and support services to individuals and their families to enhance mental health and wellbeing outcomes and reduce harm associated with substance misuse.

RESPONSIBILITIES

  • Conduct high quality comprehensive and client-centred assessments (including risk assessment) of clients presenting to the AOD service.
  • Work within a multidisciplinary team to provide coordinated care to support people to achieve their goals, including organising and/or participating in multidisciplinary care reviews and reflections.
  • Facilitate and complete referrals to other health, mental health AOD and welfare support services where appropriate (including residential withdrawal, rehabilitation, and supported accommodation).
  • Develop and implement collaborative Individual Recovery Plans and treatment plans with clients and families based on a comprehensive assessment, to support treatment goals and recovery.
  • Maintain updated and appropriately standardised documentation, encompassing client case notes, reports, and various written communications. Facilitate seamless information handover as needed.
  • Build relationships with AOD, mental health services, primary health services and other health and community, emergency, and justice related services service providers, and work collaboratively to ensure effective coordination and continuity of care for clients.
  • Participate in informal and formal case conferences, staff meetings and planned meetings.

WE NEED YOU TO HAVE

  • Relevant tertiary qualifications in a principle discipline, or Cert IV AOD/mental health competencies and relevant experience in AOD sector (required).
  • Knowledge and capacity to implement comprehensive AOD interventions.
  • Experience in liaising with other community service organisations for the development of on-going relationships and referral protocols beneficial to client support.
